How does MLB count the three-batter minimum if an inning ends early?
The minimum counts completed plate appearances, not outs recorded by any method. A plate appearance is complete when the batter is put out or becomes a runner. Under the 2026 Rule 5.10(g)(1) Comment, the pitcher on the mound when a plate appearance ends gets credit for facing that batter; for example, a reliever who enters at a 2–0 count and walks the batter on the next two balls receives that credit. If the offense is put out before a substitute pitcher completes the minimum, the pitcher may be removed between innings. A pickoff does not count as a batter faced, although a pickoff that ends the inning can permit that between-inning removal. If the same pitcher stays in the game and continues pitching in the next inning, the remaining plate appearance count carries forward. The injury or illness exception applies only when, in the umpire-in-chief’s judgment, the pitcher is incapacitated from further pitching.

Count completed plate appearances
A starting or substitute pitcher generally must face at least three consecutive batters, including the batter already at the plate when a reliever enters, until those batters are put out or reach first base. A plate appearance counts only when it ends with the batter out or becoming a runner. An intentional walk counts once completed.
The 2026 Rule 5.10(g)(1) Comment credits the pitcher who is on the mound when the plate appearance is completed. For example, if a reliever enters at a 2–0 count and walks the hitter on the next two balls, that reliever receives credit for facing the batter.
A pickoff is a separate play, not a completed plate appearance. It does not add one to the pitcher’s count.
An inning-ending out can allow removal between innings
If the offense is put out before the substitute pitcher has completed three plate appearances, the pitcher may be removed between innings. The comment specifically explains that a pickoff does not fulfill the minimum but can permit early removal when that pickoff records the final out of the inning.
If that same pitcher remains in the game and continues as pitcher in the next inning, the count carries across the break. He must complete the remaining plate appearance or plate appearances unless the offense is put out again before completion or the narrow incapacitating injury-or-illness exception applies.
The injury exception is narrow
A pitcher may be removed before satisfying the minimum if an injury or illness incapacitates him from further play as a pitcher, in the umpire-in-chief’s judgment. The rule does not make every mound visit, difficult matchup, or inning break an exception.
This guide follows Rule 5.10(g) in the 2026 Official Baseball Rules, including its 2026 comment. It describes MLB and its affiliated professional development system; amateur leagues may use different requirements. It does not resolve a real substitution or disciplinary ruling.
Two walks, then a pickoff ends the inning
In a fictional game, a reliever enters with two outs and completes two plate appearances by walking two batters. Before the next plate appearance is completed, a pickoff records the third out. The pickoff does not count toward the three-batter minimum, but the offense has been put out, so the manager may remove the reliever between innings. If he stays in the game as pitcher in the next inning, he still owes one completed plate appearance unless the offense is put out again before it is completed or the umpire-in-chief applies the incapacitating injury-or-illness exception.

Does a pickoff count as one of the three batters?
No. The minimum counts completed plate appearances. A pickoff can allow removal between innings when it records the third out, but it does not increase the batter count.
Can a pitcher be removed when the inning ends before three plate appearances?
Yes. If the offense is put out before the substitute pitcher completes the minimum, the comment permits removal between innings. If he stays in the game as pitcher, the remaining count carries forward.
What injury allows an early pitching change?
The exception is for an injury or illness that, in the umpire-in-chief’s judgment, incapacitates the pitcher from further pitching.
